Locally-owned in-home care agency with a reputation for outstanding service and culture is growing its administrative team! We are seeking a Client Care Supervisor to join our administrative team.

As our Client Care Supervisor, you will be an essential element in maintaining and growing our company's elite reputation for excellence. Through your thoughtful and professional approach, creative problem-solving, articulate communication, attention to detail, and confident leadership, you will play a key role in ensuring we maintain an impeccably satisfied clientele and an invested and motivated staff.

In our line of work, each day brings its own set of priorities, responsibilities, and unexpected challenges so the right candidate for us utilizes strong time management skills, accountability, and flexibility to gracefully take on a somewhat unpredictable and varied task list and schedule.

Essential duties for this position include:

  • Managing implementation of services for new clients, including communicating with clients’ families, coordinating with outside service providers, coordinate the RN for their role in care, and orienting caregivers to client-specific care plans;

  • Conducting regular on-site Supervisory Visits to ensure optimal client satisfaction and caregiver performance;

  • Establishing, maintaining, and regularly auditing physical and computer-based client records;

  • Strictly adhering to all Oregon Administrative Rules pertaining to in-home care agencies;

  • Contributing to the evaluation of caregiving staff, including skills, adherence to Trilogy Home Care policies, and overall conduct;

  • Responding to and resolving client and caregiver job-related needs and questions as they arise.

  • Serving as liaison between clients, caregivers, referral sources and administrators; developing and maintaining heartfelt relationships with both clients and caregivers, reinforcing established company culture

  • Assisting the RN with coordinating with physicians, preparing medication information and documentation, managing documents for compliance, including data entry etc.;

  • Contributing to Quality Assessment/Performance Improvement committee and Safety Committee efforts;

  • Accepting front-line responsibility for incoming phone calls, on an on-call rotation basis (after hours and on the weekend);

  • Required Driving, sitting, standing, lifting, bending, pulling, climbing stairs etc, without limitations; and

  • Assisting with additional projects and duties, as assigned.

 

Our ideal candidate exhibits the following qualifications:

  • Experience in a health care setting (In-Home Care Agency, Home Health Agency, Hospice Agency, Skilled Nursing Facility, Hospital, Retirement Community, Outpatient Clinic, Adult Family Home, etc.);

  • Proven ability to manage client care specifics, coordinating with outside service providers, families, caregivers, etc.;

  • Exceptional interpersonal skills; professional, compassionate, confident;

  • Articulate, refined communications skills, including in-person, oral, and written;

  • Solid organizational and time-management skills and ability to prioritize in complex situations;

  • Attention to detail;

  • Demonstrated proactive problem-solving abilities;

  • Proficiency with computers, including software such as Microsoft Office, Outlook, Excel;

  • Strong recall of detail and record-keeping abilities;

  • Problem-analysis and resolution skills;

  • High degree of ethics, awareness of maintaining confidentiality, good judgment, and common sense;

  • Demonstrated leadership experience;

  • Flexibility in scheduling work hours, including some evening and weekends;

  • Willingness to take direction, collaborate as a team member; and

  • Able to work independently, self-managed, and motivated.

  • Supervisory and/or management skills.

  • A Bachelor’s degree is preferred but not required.

  • As a condition of employment, this position requires meeting or exceeding minimum state requirements for a Comprehensive Background Check, Motor Vehicle Records check, and drug screening be conducted prior to hiring and randomly throughout the course of employment.

  • Driving clients or staff may be required at time.

  • A valid driver license and current automobile insurance is required.
