Comprehensive Services
Trilogy Home Care is licensed through the Oregon Health Authority as a Comprehensive In-Home Care Agency. With this level of licensure, the broadest of the 4 license classifications offered, we have the ability to provide the broadest scope of service available in in-home care. With nurses on staff to provide training and oversight, we have the capability to offer our clients complete Medication Services and assistance with advanced tasks, including Nurse Delegated Tasks.

An Individualized Approach to Care
Our services begin with a complimentary in-home assessment. At this initial meeting, we will listen, answer questions, and learn about your goals for our service. A member of our Client Care Team will work with you to identify the specifics of your needs, encompassing the whole person - the physical, social, cognitive, and spiritual. Together, we will develop a comprehensive Plan of Care. With your guidance and our assessment, we will determine the right level of care, the right schedule, and an individualized Plan of Care to ensure your goals are met.

Our Caregivers are available to you for:

As you need us service - minimum of 4 hours per visit

Up to 24-hour care provided by day and overnight caregivers.
